Nursing Care Responsibilities




  • Available to assist with elder personal care, if needed, according to plan of care and elder wishes

  • Explains procedures to elders before administering and plans care delivery to honor elder preferences

  • Performs medication pass and documents per policy

  • Assesses and documents changes in elder condition; report changes to supervisor/MD within the shift that it occurred

  • Assists in the completion of the care plan and MDS; attends care conference and maintains ongoing positive communication with families

  • Completes daily, weekly and monthly charting accurately and timely per policy; reports concerns to the nurse leader

  • Monitors skin integrity; observes elders daily or gets report from stna?s or CC?s about any skin problems; provides treatments as ordered; measures and documents on any open areas per policy

  • Implements fall reduction program on the household; initiates fall interventions in the care plan immediately after a fall; meets with staff and nurse leaders as soon as possible after a fall

  • Implements the bowel and bladder continence programs on the household per policy and restorative program; monitors stna?s for proper use of incontinence products and retraining programs.

  • Attends household meetings to address skin integrity, fall reduction, continence, nutrition and hydration

  • Makes rounds with physicians on the household when available to establish a good working relationship.

  • Completes post-discharge plan of care and educates elders on discharge needs

  • Responsible for overall sanitation of assigned work area.? Monitors cleanliness of elder rooms, central bathing area, treatment and supply rooms, workrooms and linen closets; ensures bedpans, urinals, commodes, wash basins and emesis basins are cleaned by staff after use; ensures staff sanitize bath tub & shower chair after each elder?s use; ensures floors are clean and dry.

  • Establishes and maintains good public relations with elders, families and other guests, shows empathy toward elders

  • Ensures proper post-mortem care is provided




Nutrition and Hydration Responsibilities




  • Monitors all meal times; ensures that elders are positioned appropriately; have the appropriate utensils; are served the proper diet and offered substitutes

  • Assists with serving and feeding

  • Ensures elders receive proper hydration at meals and in-between according to policy and elder needs.

  • Monitors nutrition & hydration program; documents on adequacy of elder food and fluid intake and weight loss/gain

  • Ensures restorative dining programs are carried out and documented accurately by staff






Admission, Transfer & Discharge



  • Meets with new elders immediately upon admission to identify any problems or concerns; checks on elders on the household throughout the work day.

  • Completes admission assessments and documentation per policy, accurately and timely

  • Makes elders comfortable and orients to the household and routines

  • Ensures elders? personal possessions are inventoried and documented




Supervisory Responsibilities




  • Participates in and receives elder nursing report; gives instruction to nurse aides regarding personal care issues.

  • Is aware of current policies and procedures

  • Assigns staff to work duties; adjusts schedules if necessary; supervises stna?s or CC?s on scheduled shift

  • Counsels and disciplines stna?s? or CC?s for problems with conduct, performance or reliability

  • Has the authority to send a staff member home pending investigation if there are problems with performance, conduct or a possibility of elder abuse

  • Initiates action plans for correction of identified problems in elder care delivery or to make improvements in processes




RN?s Only Responsibilities




  • Demonstrates expertise in the care of the elder with a central line (flushes, infusions, monitoring, tubing and bag changes, and mixing of meds).

  • Complies with the State regulations specific to RN?s on the insertion, administration and care of IV?s

  • Responsible for completing other tasks that only can be accomplished by RN?s per facility policy



Job Requirements




LPN Charge Nurse



Education/Experience:? Graduate of an accredited college or school of nursing; previous long-term care experience preferred.? Must be free from illegal use of drugs and free from use and effects of use of drugs and alcohol in the workplace. Must pass drug screening and criminal background checks prior to hire.? The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
